The form KND 1110021 has three sheets: the first sheet is the title page, the second one reflects information about the cash register, the third contains information about the address where the cash register is installed (cash register).

Application for registration of a cash register, form 1110021, is used not only when registering a new cash register, but also when re-registering and deregistering. Both individual entrepreneurs and organizations fill out the application.

Procedure for filling out application 1110021:

In the TIN field, enter 10 digits for organizations and 12 digits for individual entrepreneurs.

Checkpoint - filled out only by organizations. Individual entrepreneurs do not fill out this item. Attention - indicate checkpoint, not KOP (unit code).

The type of document is filled out depending on the purpose of submitting the application.

The following codes exist:

  • Registration or re-registration of a cash register: put the first one and the rest are twos.
  • Deregistration of cash registers: first three, the rest two.
  • Replacement of EKLZ: the first three twos, then one and again two twos.
  • Changing the central station: the first two twos, then one and three twos.
  • Changing the installation location address: two, then one, the rest are twos.
  • Replacing the FP block: four twos, then one and two.
  • Other reasons: all twos, the last one.

The name of the organization or individual entrepreneur must be filled in completely. For example: “limited liability company “Crystal”, the same goes for individual entrepreneurs - the full name must be entered.

OKVED must indicate the main one. You can find it in your extract from the Unified State Register.

Who provides the completed application KND 1110021:

  • if this is a legal entity, then put one in the field;
  • if it’s a separate subdivision, put two;
  • if it is an individual entrepreneur, we give it a three.

You can enter any phone number at your discretion.

In the section “I confirm the accuracy and completeness of the information,” information about the applicant is filled in:

You need to insert the required number: 1 - applicant - user (individual entrepreneur or director of an organization) 2 - applicant - representative of a particular organization or individual.

In the field about the applicant, you must enter the surname of the chief director, individual entrepreneur or the surname of the trustee.

For an authorized person - fill in the details (series and number) from the passport or number and when it was issued.

On the second sheet of the application for registration of cash register equipment, information about the model of the cash register device is filled in, for example: Mercury170K, Mercury120K, etc.

The serial number of the device is indicated - located on the KKM body.

The year of manufacture can also be found on the device itself.

The version number of cash register equipment can be copied from the passport or it may not be there at all, depending on the brand of the device.

The series with the account number is located at the bottom of the cash register. Numbers began to be issued to KKM after 2008.

Passport control number - you will find it in your passport for the KKM.

The EKLZ number is located on the tape in the passport.

The “Payment terminal” field is filled in only by those persons who use cash register systems to accept money on the street, in stores.

ECLZ service life: 12 months + 1 month for replacement (total maximum 13 months). ECLZ is blocked on the 1st (calendar) day of the 14th month from the start of operation. Starting from the 11th month of the EKLZ service life, the cash register prints a reminder in Z-reports about the upcoming replacement: " ECLZ is close to filling".

Important!

We strongly recommend that you resolve issues related to replacing the ECLZ unit in advance (exactly 12 months after activating the ECLZ), without waiting until the cash register is blocked and the cash register will be much more downtime in operation. We also draw your attention to the fact that the planned replacement of EKLZ units will affect all cash registers registered with your Federal Tax Service, and therefore queues may form in the first days of the month.

Separately, we would like to remind customers who registered their cash registers in November: cash register will be blocked on January 1 , therefore, the inspection will be able to give permission for replacement only after the New Year holidays. It is advisable to pay for services related to the replacement of the ECLZ unit in advance, since the number of ECLZs in the reserve of the central heating station is limited.

You can fill out an application in the Russian Federal Tax Service program “Legal Taxpayer”.

Can be done by hand. In this case, you can add the missing numbers and letters in the fields that do not have the required number of cells.

If you fill out an official form on a computer, then, as mentioned above, they have a limited number of cells for some lines. Let's say there are 12 cells, and the passport number contains 15 characters.

And it will not be possible to correctly complete the missing number of characters either on a computer or with a pen (since mixing both a printed version and a pen is unacceptable).

KND 1110021 is an application form that is used both upon first registration with the tax authority and every year thereafter.

This document contains several fields with the following information:

  1. Field to indicate identification number but an organization or an entrepreneur. This information is confirmation of delivery for registration with the Federal Tax Service. Attention! For legal entities, the identifier code will consist of 10 digits, and the field to fill out has 12 spaces, so two boxes must be marked (dash).
  2. "Checkpoint". Information is provided exclusively from a legal entity or its separate division. For entrepreneurs, there is no need to fill out the column.
  3. "Tax authority code". It is necessary to enter the code of the tax service to which the enterprise or entrepreneur belongs. To find out your tax authority code, you can use this.
  4. "View". Characterizes the type of document. A field consists of several cells that display specific information. The first cell is required to indicate the code corresponding to a specific reason for submitting a document (1 – initial registration, 2 – the need for re-registration, 3 – the need to deregister the device). The next five cells are used to enter reason codes for re-registration of cash registers. In these cells you can enter a code “1” and a code “2”, which indicate the answers “yes” and “no”, respectively. If initial registration is carried out or the device is deregistered, then code “2” must be entered everywhere. Examples:
  • initial registration – 1 / 2 2 2 2 2;
  • re-registration due to a change in service center – 2 / 2 1 2 2 2;
  • linking a cash register to another address (change of address for using the device) – 2 / 1 2 2 2 2;

  1. "Name". The enterprise is required to indicate its full official name, the entrepreneur - full name (according to the identity document).
  2. “KVED or codes of types of economic activity, according to the state classifier”. This field is filled in according to the list. To find their activity code, they can look it up in their statement or Unified State Register of Legal Entities, respectively.
  3. "Provided". Responsible for information regarding the person submitting the application. These can be separate divisions and organizations, companies or entrepreneurs themselves.
  4. "Contact number". There is for specifying a phone number (mobile or landline).
  5. "Number of pages". It is necessary to indicate how many pages the document consists of (most often “003”).
  6. "Application". The number of sheets of each document that was attached to the application is indicated. In cases where such documents are not available, a dash is added.
  7. Block “I confirm”. This part contains information confirming the completeness of the power of attorney information contained in the document. In this block it is necessary to enter the personal data of the entrepreneur or the head of the organization line by line. Indicate the date of preparation and submission of the paper, affix your signature and stamp (if necessary). In the case when the declaration is submitted by a representative, additional information is required that confirms the authority of this person, as well as attaching this document to the application itself.

All other fields on the Title Page will be filled out by a Federal Tax Service employee.

Section No. 1. Device model information

All necessary information about the device model is indicated. The section consists of several fields and lines:

  1. Fields "KPP" and "INN".
  2. "010"– the model of the cash register device is indicated, which is written on the card when registering the device, and is also, as a rule, on the case and in the registration certificate.
  3. "020"– to indicate the serial number of the cash register. It is also indicated in the technical data sheet, on the card or on the case.
  4. "030"- to indicate the year of manufacture. This information is also contained in the registration card, technical data sheet or on the case.
  5. "040"– to indicate the device version number, if available. It is placed on the identification mark of the device or in its technical passport.
  6. "050"– to indicate the account number of the cash register identification mark indicated in the technical passport or on the case.
  7. "060"– to indicate the passport number.
  8. "070"– designation of the EKLZ serial number (under the barcode on the EKLZ passport).
  9. "080"– writing down the registration number of the EKLZ (after activation of the EKLZ in the receipt or in the appendix to the device passport). If an application is submitted for registration, the column is not filled in.
  10. "090"– indication of information about the inclusion of a cash register device in the payment terminal set (for example, terminals located in stores, or ATMs, etc.). This line contains two codes: “1” (“yes”) and “2” (“no”).
  11. "100"– to determine the internal number when registering a payment terminal. Filled in if the previous line is marked “yes” (code “1”).
  12. "110". Filling is required when “yes” is indicated in line “90”. This is the number of the payment terminal.
  13. "120"– a note is made by a tax officer only in cases where the line “090” is marked “yes”. The code for accessing the Federal Tax Service inspector mode is written here.
  14. "130". A line to indicate the name of the technical service center where the cash register service agreement was concluded.
  15. "140"– it is necessary to indicate the identification number of the maintenance center. You can find it in the center itself or look at it in the concluded agreement.
  16. "150"– the number of the contract with the service center is written down, and the period of its validity is also indicated. Typically, the start date and end date of its operation.
  17. "160"– registration TIN, brand of seal adhesive to the device body. The first part of the column indicates the number located in the upper half of the sticker, the second part indicates the number of the lower half of the sticker.
  18. "170"– information about the number and years of manufacture of the device. Usually they are indicated on holograms glued to the case, or in the device passport.

This information must be dated and signed.

Section No. 2. Data on the location of the CCP installation

  1. "KPP" and "INN". These fields are filled in similarly on all documents.
  2. Exact device installation address. The type of outlet and its name, if any, must be indicated. Examples: DOLE store, office 120, Children's Toys kiosk, Let's Fly Far Away travel agency, Elvira beauty salon.
  3. In the event that the premises where the cash register is located are rented, it is important to indicate the following information: the title or name of the person who rented the premises; validity period and number of the rental agreement; Landlord identification number.

In cases where cash register equipment is part of a payment terminal or ATM, it is important to indicate the following data:

  1. The name of the company or person with whom an agreement has already been drawn up to accept payments.
  2. Company identification number.
  3. Contract data (validity lines, number).

Replacing EKLZ: the procedure for 2016 is not regulated by law. For companies, replacing ECLZ often causes difficulties. We have discussed the procedure in detail.

Replacement of EKLZ procedure for 2016 is not regulated by law. There is no exact deadline for submitting an application to the tax office, but it would be better to submit the application simultaneously with the notification of the central service center about the replacement of the EKLZ.

Situation: Replacing EKLZ procedure 2016

ECLZ must be replaced in the following cases:

  • when the ECLZ memory is full;
  • upon expiration of the established service life of the EKLZ (regardless of the degree of memory fill);
  • when re-registering a cash register;
  • in the event of a malfunction of the EKLZ unit (repair of the EKLZ both within the CCP and outside the CCP is not allowed).

Work related to the replacement of EKLZ can only be performed by suppliers and central service centers. Organizations (entrepreneurs) do not have the right to change EKLZ independently. This is stated in paragraph 3.1 of the Methodological Instructions approved by the GMEC protocol of June 25, 2002 No. 4/69-2002.

The organization (entrepreneur) is obliged to promptly inform the central service center about the need to replace the EKLZ in the cases listed above (clause 10 of the Regulations approved by decision of the GMEC of March 6, 1995 No. 2/18-95).

The procedure for replacing ECLZ is not regulated by law*. According to the explanations of the financial department, before the adoption of regulations on this issue, one should be guided by the temporary clarifications of the tax service.

If the EKLZ is replaced, the procedure for 2016 is as follows:

  • notify the central service center one month before the expiration date;

If the cash register has issued a message that the ECLZ memory is more than 90 percent full, you must immediately:

  • notify the central service center about this;
  • send a letter of guarantee application to the central service center for the installation and activation of a new ECLZ.

Before replacing the ECLZ, you must:

  • make a short report for the period from the moment of activation of the ECLZ to the last closed shift;
  • take the results of the current shift;
  • close the archive in accordance with the operational documentation of the cash register.

In this case, you need to submit to the tax office:

  • application for registration of a cash register (clause 16 of the Regulations approved by Decree of the Government of the Russian Federation of July 23, 2007 No. 470). It indicates the serial number of the new EKLZ unit, as well as the number of the current KKT passport (until the approval of a new sample KKT passport), and in the absence of a passport, a dash is placed (letters of the Federal Tax Service of Russia dated April 3, 2012 No. AS-4-2/5537 and dated March 21, 2012 No. AS-4-2/4767);
  • CCP registration card on which the EKLZ will be changed;
  • form No. KM-4;
  • technical conclusion of the technical center on the reason for replacing the ECLZ;
  • act in form No. KM-2, certified by the seal of the Central Technical Service (drawn up in two copies). The report must reflect the readings of control meters before replacing the EKLZ. The act in form No. KM-2 is filled out in the presence of tax inspectors (instructions approved by Resolution of the State Statistics Committee of Russia dated December 25, 1998 No. 132, letter of the Federal Tax Service of Russia dated September 20, 2011 No. AS-4-2/15390).

If the EKLZ is replaced, the procedure for 2016 and the documents are as follows:

  • KKT passport. In this case, in the column “Special notes” the date of replacement and activation of the EKLZ must be indicated, indicating its registration number;
  • additional sheet to the KKT passport with a mark on the activation of the EKLZ;
  • act of putting EKLZ into operation;
  • journal in form No. KM-8.

The listed documents are prepared by the center that carried out the replacement of the EKLZ and transferred to the organization (entrepreneur) for submission to the tax office.

After checking the documents, the tax office returns them to the organization (entrepreneur). At the same time, the inspection must return:

  • CCP registration card with a mark about the activated ECLZ;
  • cashier-operator's journal
